The defendant is charged with the crime of negligent homicide in operating his motor vehicle in such a manner as to cause the death of [name deceased]. To prove this charge, the prosecutor must prove each of the following elements beyond a reasonable doubt:

First, that the defendant was operating his motor vehicle on or about [date], at [place].

Second, that he operated the vehicle at an immoderate rate of speed or in a careless, reckless, or negligent manner.

Third, that the defendant's negligent driving was a substantial cause of an accident resulting in injuries to [name deceased].

Fourth, that such injuries caused the death of [name deceased]
